The Western Wayne School District will be offering free sports physicals for any student grades 6-11 who may be interested in playing a sport during the 2025-2026 school year on Wednesday, May 7th, 2025.
Frequently Asked Questions:
Q. Where can a student find the PIAA Physical Form?
A. Forms can be picked up in the High School Main Office, Middle School Student Waiting, Athletic Office, Online
Q. Does a student's physical from this school year work for next school year?
A. No. For a physical to be good for the 25-26 school year, all forms must be dated after May 1st, 2025. Any forms dated before this date will not be sufficient.
Q. If a student plays a fall sport, do they need to see the doctor again if they want to play a winter or spring sport?
A. No. If student has a PIAA Physical on file, all they will need is a recertification form which can be filled out by a parent or guardian. They would need a Recert for both the Winter season and Spring Season.
Some important information to keep in mind:
A student must have a PIAA Physical in order to participate in sport. A regular physical examination form may be different than a PIAA form and will not be sufficient to play sports.
Sections 1-5 must be completed and handed in to the school by Monday, May 5th in order to see the doctor on May 7th.
Free sports physicals will also be offered prior to each sports season (Fall, Winter, Spring)
If a student misses the free sports physical offered by the school, they will need to see their primary care doctor or go to an urgent care.
A PIAA Physical IS REQUIRED before a student can participate in sport.
