Free Sports Physicals -- Winter

There will be Free Sports Physicals on Tuesday, November 4th for any student grades 7-12 who are interested in playing a winter sport and have not yet had a physical.
Attached is the PIAA physical form. Sections 1-5 must be filled out and handed in prior to November 4th in order to see the Doctor.
Physical forms can also be picked up in the following locations:
- Athletic Office
- Middle Schol Student Waiting
- High School Main Office
